Fit (Sick) Note – Self Certification

Fit (Sick) Note – Self Certification. If you are off work for 7 days or less, please do not ask us to provide a fit note for your as you do not need to give your employer a fit note or other proof of sickness from a healthcare professional.

When you return to work, your employer may ask you to confirm you’ve been off sick. This is called ‘self-certification’. You will agree with your employer on how you should do this. You might need to fill in a SC2form, which can be found here.

Fit Notes can only be issued by us for a period of illness that you have consulted us for.  Post procedure fit notes and extensions to them are issued by your consultant and requests for these should not be directed to the surgery as we are unable to assist.
