Register as a new patient at Spilsby Surgery
We are using a new online service called Register with Spilsby surgery that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper registration forms
Paper forms are still available here if you need one. You can print off, complete and hand in to reception at the surgery or come to the surgery and collect a form.
Form 1 – NHS Registration Form
Form 2, 3 and 4 – Spilsby Surgery Medical Registration and Health Information Form
We ask you to complete the health questionnaire at the time of registration. This questionnaire will include questions about lifestyle and medical history and must be completed alongside the NHS registration form in order to register.
If you wish to gain online access to your medical records, we will need to see you in person with some photo ID to verify your identity. This is due to the confidential information accessible on the online access platforms.
Please return completed forms to us by email if you can, alternatively you can hand them in at the surgery.
Form 1 – must be completed for all registrations
Form 2 – must be completed to register – ADULT
Form 3 – must be completed to register – CHILD
Form 4 – must be completed to register – NEW BORN
